- Retain 100% of exam and professional fees
Optional Services (for a fair market value):
- Equipment and instrumentation
- Administrative and/or Opt-Tech support services (where allowed by state regulations)
Minimum Qualifications for Lease Agreement:
- Doctor of Optometry (OD) from an accredited optometry school with valid state optometry license, OR
- Ophthalmologist (MD or DO) from an accredited medical school with valid state ophthalmology license
- General Liability Insurance Certificate
- Professional Liability Insurance Certificate
- Completed W-9 form
